answers
common questions our new patients have. We hope you will find this information
helpful and reassuring. Should you have any further questions, please

Please arrive 15 minutes
prior to your appointment time. Doing so will allow the front office staff
to process your information and get you in to see the doctor on time.
Your first appointment will
last for about 1-1/2 to 2 hours. Subsequent visits usually last around 20 to
30
minutes.
Bring your insurance
card(s) with you for every visit. We will photocopy them before each appointment.
This has proven to be the most effective way to follow all of the rules that
insurance companies require of us.
Also, be aware of the co-pay
amount, or percentage total, that your insurance company requires,
and be prepared to pay. We accept cash, checks, and VISA, MasterCard
& Discover credit cards.
It is your responsibility
to obtain a REFERRAL NUMBER from your primary care physician, if it is
required
by your insurance company.
Bring all medications you are currently taking.

to the amount of time set aside for your visit, we ask that you give us 24 hours
notice of
cancellation or be charged a possible fee.
